MediShield Life and Integrated Shield Plans in Singapore
How Singapore's health insurance system works for residents.
Singapore operates one of the most structured healthcare financing systems globally, built on the '3M' framework: Medisave (mandatory health savings), MediShield Life (universal basic health insurance), and Medifund (safety net for the needy). MediShield Life is a basic health insurance plan that all Singapore Citizens and Permanent Residents are automatically enrolled in. It covers large hospital bills and selected costly outpatient treatments like dialysis and chemotherapy at Class B2/C wards in public hospitals.
Premiums are age-based and paid from Medisave, ranging from S$130/year for those under 30 to S$2,350/year for those above 90. While MediShield Life provides basic coverage, most Singaporeans supplement it with Integrated Shield Plans (IPs) offered by private insurers. These upgrade your coverage to Class B1 or Class A wards in public hospitals, and to private hospitals.
The seven approved IP providers are AIA, AXA, Great Eastern, Income, Manulife, Prudential, and Raffles Health Insurance. Each offers plans at different ward tiers: B1 ward plans cost approximately S$300-600/year for a 30-year-old, A ward plans S$500-900, and private hospital plans S$800-2,000. Key features to compare across IPs include the panel of private hospitals, co-insurance percentages (typically 5-10% for IP claims), annual and lifetime claim limits, pre and post-hospitalisation coverage duration, and whether the plan covers as-charged or has procedure-specific sub-limits.
The Monetary Authority of Singapore (MAS) and the Ministry of Health (MOH) jointly regulate health insurance. MOH sets the framework for IPs while MAS regulates the insurers. IP riders that cover the co-insurance and deductible portions were restricted by MOH in 2018 to discourage over-consumption of healthcare.
